Smile Builders Dental Program

SmileBuilders is coming to YOUR school! Click here for schedule.

Our skilled Dental hygienists from StayWell Health Center are going to schools in Waterbury and Naugatuck to provide dental services for your children throughout the year. We provide services such as cleanings, dental screenings, fluoride treatments, and sealants to prevent decay on adult molars. These services are provided for your convenience, so that means NO missed work and NO missed school days for your kids!

We work with your insurance and for those who are uninsured; we offer our services at a LOW and affordable cost. Any further treatment needed can be followed up at StayWell in order to give your child the care that they deserve. Forms will be given out in the beginning of the school year and are also available in the nurses’ office. If you have any questions please call 203-756-8021 ext. 3815.

During the summer we came up with a different form for taking days off that is earth friendly (electronic).  There are only 2 forms now: the form for Professional leave and the form for Personal, Funeral, Family Sick, Religious, Jury Duty, and Vacation (SAW).  This is the only form that will be accepted.  You may dispose of the old forms as you see fit. The form is located on the Waterbury.k12.ct.us website under staff, then HR Forms. It is called: Request to be Absent Form, for all employees.  The new form is electronic.  You need to fill it out and electronically sign it, email it directly to your principal for them to electronically sign it, and they will electronically forward it to the HR department for final approval.  There is a separate email that they need to go to.  It is hrrequest@waterbury.k12.ct.us.  If you have an issue with signing it electronically please contact your IT department for an update.  Our hope is to cut down on the paper usage, the wear and tear on the fax machines and copy machines, and the pricey toner cartridges.
