While our lottery for entrance in the 2023-2024 school year has officially ended, we are always accepting paper applications for all grade levels (6-12). These applications are highly regarded and sought. Simply complete the application listed below and return to Waterbury Arts Magnet School to the attention of Rhiana Murphy. This is the form for the PTSO Ice Cream Pie fundraiser.

This form should be submitted when a parent does not wish for their child to participate in media coverage.

This form is used for student physicals and sports physicals.

This form is for a non-WAMS student to attend a WAMS event as a guest.
