Parents/Guardians:

If you wish to withdraw your student(s) to homeschooling, please complete and return the notice of intent to homeschool to either of the following:

  1. In person at your student(s)' current school.
  2. In person at the Chase Building located at 236 Grand St. Room 161 Waterbury, CT 06702 or
  3. Certified Mail to:
